Cements for Filling and Luting Applications

In dentistry, cements are used either as restorative filling materials or luting cements for the placement of dental prosthetics. Compared to amalgam or composite fillings, dental cements offer advantages such as a favorable cost-performance ratio, coverage by statutory health insurance, and easy application.

They are also versatile, for example as temporary fillings or provisional cements for securing temporary crowns or bridges. In addition to traditional zinc phosphate cement, there are now many other types of cements, including zinc polycarboxylate cement, glass ionomer cement, and resin-modified glass ionomer cement.

Dental Cement as a Base Material

In restorative dentistry, dental cement is often used as a base layer beneath composite, amalgam, or other restorative fillings in teeth affected by caries.

For this purpose, the carious dentin is removed, the cavity is fully cleaned of decay, dried, and the cement is placed into the cavity. Dental cement has also been used for decades as a filling material itself, for example in primary teeth or as a build-up material for prosthetic restorations.

If a tooth is intended for later restoration with an inlay or similar, it can be temporarily filled with dental cement, as this type of filling can later be easily removed.

The cement can be mixed manually from powder and liquid (e.g., Hoffmann's Carboxylate Cement Powder) or used in convenient pre-dosed capsules (e.g., Harvard Cement OptiCaps).

Cement as a Luting Material

Dental cement remains one of the most commonly used materials for the cementation of crowns, bridges, or ceramic restorations. The powder and liquid are mixed on a cool, dry surface using a metal spatula. Glass slabs are particularly suitable as a mixing surface, as they dissipate the exothermic reaction and prevent premature setting.

Once the correct consistency is achieved, the mixed material can be easily placed into cleaned and dry crowns and the restoration can be seated. The set luting cement can then be removed cleanly and without residue from the tooth, gingival margin, and bridge.

Alternatively, pre-dosed capsules, such as Harvard MTA XR OptiCaps, can be used for the cementation of fixed restorations like bridges, crowns, and inlays. The material in OptiCaps is easy to dose and can be mixed homogeneously and bubble-free using capsule mixing devices.

Dental cement can also be applied directly from the capsule into the cavity or crown. For the integration of partially transparent crowns in the visible area, luting cements are now available in various shades, allowing a customized match to the prepared tooth and adjacent teeth.

Patients can usually chew again shortly after treatment, and the restoration is immediately load-bearing. If a restoration is intended to be temporarily cemented, provisional dental cement is ideal. Despite its excellent adhesive properties, the restoration can be easily removed at any time.
