Because of the fine vascular structure of colon carcinomas or colorectal polyps, it often comes down to blood additives in the stool that do not considerably change the appearance of the stool to a great degree and are therefore, very difficult to recognize with the eye, alone.

Detection of hidden blood can be carried out with a chemical or immunological stool test. Chemical stool tests detect blood in stool by way of the enzymatic activity of the red blood cells, whereas immunological stool tests base the detection on human hemoglobin via a specific antibody reaction.

Opposite to the haemoccult test, this style of stool test has no risk of false diagnosis from certain medications or food stuffs (e.g red meat).
